UPMC Pinnacle has been taking a more active role in helping members of your community stay healthy. The steps to accomplish this involve your ministry, on behalf of your congregants/members and ministries, to join the Faith Community Health Connection (FCHC). Being a member of the FCHC allows our covenant community partners preference to receive a UPMC Pinnacle Community Health Navigator at their community event.
Rev. Dr. Brenda Alton oversees the UPMC Pinnacle Faith Community Health Connection (FCHC) and requests from the faith community for a Community Health Navigator to attend your community event. Attached you will find:
The FCHC is a free initiative based on the successful Congregational Health Network program from Memphis TN, where hospitals and trained volunteers in faith community’s work together to better serve ailing members.
Benefits of joining the FCHC for Congregations:
- Know right away when someone in the congregation is admitted to the hospital if your congregant consents to have you notified.
- Deploy existing ministry resources more efficiently.
- Provide meaningful volunteer opportunities and ongoing training to promote the health of the congregation.
- Access free health promotion resources to promote the health of your congregation.
