2026 Narrative Report Form

On behalf of this synod and the Evangelical Lutheran Church in America, thank you for your partnership in the Gospel of our Lord and Savior Jesus Christ. Thank you also for your patience with receiving the annual report form. The complete Report to the Synod Bishop is sent out in printed form via US Postal mail. Recipients of the report may wish to fill in their narrative report page using an electronic device. This page has seven different versions which are defined by the current position of the rostered minister. The page is in a fillable PDF file which you may download by clicking on the link below that describes your current position.

After completing your entries, save the file, print the page and return it to the synod office, along with any other pages that are part of the original report by June 6, 2026, or bring with you to the Synod Assembly where we will have a place to deposit your report onsite.
